Hyaluronic acid butt filler or biostimulator: what is the difference, and which should you choose?

In butt filler, body hyaluronic acid (UPmax, Sofiderm) delivers volume and projection right at the time of injection. A collagen biostimulator (Sculptra, Radiesse) is not meant to increase size: it stimulates collagen and improves firmness and skin quality little by little. Many buttock filler plans combine the two. What decides is your goal, not the brand.
What is the difference between body hyaluronic acid and a biostimulator?
The difference lies in what each product does inside the tissue.
Body hyaluronic acid is a dense gel that takes up space. When it is placed in the buttock, it fills: the volume appears the same day. The body versions, such as UPmax and Sofiderm, are formulated for use on the body, in large areas, and are different from those used on the face. Over time, the body absorbs the product, which is why the result calls for maintenance. By stretching the tissue, it may also stimulate some collagen — but that is a secondary effect, and it does not make it a biostimulator.
A collagen biostimulator works differently: instead of taking up space, it triggers a response from your own body, which starts producing new collagen around the material. The result is firmer skin and better texture, which appear gradually. The two best known are Sculptra (poly-L-lactic acid) and Radiesse (calcium hydroxylapatite) — different molecules, but from the same family of action.
A frequent source of confusion: UPmax and Sofiderm have names that sound like biostimulators, but they are hyaluronic acid. It is worth confirming this with whoever is going to inject.
Comparison table: hyaluronic acid, biostimulator, combination, and your own fat
| Body hyaluronic acid | Biostimulator | Combination | Your own fat (Lipocube) | |
|---|---|---|---|---|
| Examples | UPmax, Sofiderm | Sculptra, Radiesse | Hyaluronic acid + biostimulator | Fat removed from your body |
| What it is for | Volume and projection | Firmness, sagging, cellulite, texture | Volume with firmer skin | Volume with tissue from your own body |
| When the effect appears | At the time of injection | Little by little | Volume right away; firmness little by little | Defined at the consultation |
| How it is charged | Per cycle, according to the volume | Per box | The sum of the two calculations | Per protocol |
| Range with Dr. Thiago Perfeito, CRM-DF 23199 (2026) | R$ 18,000 to R$ 55,000 per cycle | R$ 2,900 to R$ 3,900 per box; at least 1 box on each side; the ideal is between 2 and 4 on each side | The sum of the ranges | R$ 30,000 to R$ 50,000 per protocol |
The amounts are ranges, in Brazilian reais (R$): the quote for your case comes out of the consultation. The pricing details are in butt filler cost.
What is your goal: volume, lift, hip dips, or sagging skin?
Answering this question settles half of the choice.
I want more volume
The route is body hyaluronic acid. A biostimulator alone will not make the butt grow noticeably. How much product is used depends on your starting point — see how many mL.
I want a more lifted butt
Here the design matters more than the quantity: the hyaluronic acid is positioned in the upper part of the buttock to change the projection. When the skin has already lost firmness, a biostimulator usually comes in alongside it, to provide support.
I have hip dips
The depressions on the sides of the hips, when filler helps, are treated with body hyaluronic acid in a localized injection. A biostimulator does not fill a concavity. Read the page on hip dip filler.
What bothers me is sagging skin or cellulite
This is biostimulator territory. The amount goes from a minimum of 1 box per side up to 2 to 4 per side as the ideal; in significant sagging or cellulite, it can reach 5 or 6 boxes per side.
When is it worth combining the two?
Combining makes sense when you have both problems at the same time: volume is lacking and the skin is less firm. A common example is the buttock that deflated after weight loss — it lost its filling and, along with it, its support.
In that scenario, the hyaluronic acid restores contour and the biostimulator works on the quality of the skin that covers that contour. The final amount is the sum: the hyaluronic acid cycle plus the boxes of biostimulator. The order of the steps and the interval between them are defined in the plan.
And your own fat — when does it come in?
Lipocube is the grafting of your own fat: it is removed from an area where there is excess and placed in the buttock. It is done under local anesthesia. It is of interest to those who have fat available and prefer to use tissue from their own body instead of a product. The range is R$ 30,000 to R$ 50,000 per protocol. Whether it makes sense for you becomes clear at the consultation, when the available fat is examined.
And technologies such as Morpheus8?
Technologies such as Morpheus8 do not fill: they act on the skin of the buttocks and thighs, as a complement in cases of sagging or texture. With Dr. Thiago, a session for buttocks and thighs ranges from R$ 6,000 to R$ 12,000. They enter the plan only when there is an indication.
Which mistakes should you avoid when choosing?
- Expecting volume from a biostimulator. It improves firmness; for size, the product is a different one.
- Comparing the price of a box with the price of a cycle. A biostimulator is charged per box and hyaluronic acid per cycle: they are different units.
- Choosing by brand. The brand comes after the goal.
- Accepting a permanent product. PMMA is not an alternative to either of the two; Dr. Thiago does not use it. Understand why in safety and PMMA.
- Not seeing the box. With Dr. Thiago, the boxes are shown and opened in front of you, and the lot labels go into your chart.
