How does butt filler work, and who is it for?
Butt filler — also called buttock filler, a non-surgical BBL, or buttock augmentation without surgery — is an injectable procedure performed in the office under local anesthesia. It uses body hyaluronic acid when the goal is volume and projection, or a collagen biostimulator when the goal is firmer skin. There is no incision. The product, volume, and price for your case can only be defined at an in-person consultation.
What exactly is butt filler?
Think of butt filler as a way to reshape the contour of the buttocks without surgery. Instead of an implant or an incision, the physician injects a product at planned points, either to restore volume where it is lacking or to improve the quality of the skin that covers the area.
This guide was written by Dr. Thiago Perfeito, physician (CRM-DF 23199), who practices Aesthetic and Regenerative Medicine and has more than 500 buttock cases. The purpose here is simple: to explain each step in plain language, so that you arrive at your consultation knowing what to ask.
One distinction helps from the start. Filling is not the same as increasing size the way an implant would. Filler improves contour, projection, and proportion; if you are expecting a change of a different order of magnitude, that needs to be discussed frankly at the consultation.
Which techniques exist: hyaluronic acid, biostimulator, or your own fat?
There are three main routes, which can be used alone or in combination:
- Body hyaluronic acid (UPmax, Sofiderm): a high-density gel made for the body. It is what provides volume and projection, and the effect is visible right at the time of injection. A note on the names: although they may sound like biostimulators, UPmax and Sofiderm are hyaluronic acid.
- Collagen biostimulator (Sculptra, based on poly-L-lactic acid; Radiesse, based on calcium hydroxylapatite): it is not meant to make the buttocks bigger. It stimulates your body to produce new collagen, which treats firmness, sagging, and texture, with gradual results.
- Your own fat (Lipocube protocol): fat is removed from one area of your body and grafted into the buttock, under local anesthesia.
In the detailed comparison, hyaluronic acid or biostimulator, you can see which one makes sense for each goal.
What is your goal: volume, lift, hip dips, or sagging skin?
The best place to start is not the brand of the product, but the question: what, exactly, bothers you in the mirror?
| Your goal | Most common route |
|---|---|
| More volume and projection | Body hyaluronic acid |
| A more lifted butt | Body hyaluronic acid at strategic points in the upper buttock, often combined with a biostimulator for support |
| Hip dips (an indentation on the side of the hip) | Body hyaluronic acid placed only in the area of the depression — see the page on hip dip filler |
| Sagging skin, cellulite, thin skin | Collagen biostimulator |
| Volume and firmness at the same time | A combination of the two |
This table is a map, not a prescription. The same wish can call for different routes in different bodies.
Who is butt filler a good fit for — and who is it not?
It tends to make sense for people who want to improve the contour of the buttocks without going through surgery: those who have lost volume over time or after weight loss, those with one side different from the other, those who feel their buttocks look flat, or those bothered by looser skin.
Some situations call for caution or a different conversation:
- Anyone who has already received PMMA or another product in the buttocks needs to say so right at the consultation, because it changes the entire plan. Dr. Thiago does not use PMMA.
- Pregnancy and breastfeeding are times to postpone injectable aesthetic procedures.
- Anyone expecting an increase equivalent to that of an implant will probably be disappointed with any filler.
How many mL of butt filler are typically used?
In Dr. Thiago's stated practice, body hyaluronic acid in the buttocks usually ranges from 30 to 90 mL in total:
- smaller adjustments: 30 mL for both sides combined, or 15 mL on each side;
- medium adjustments: 60 mL, or 30 mL on each side;
- larger projections: 90 mL, or 45 mL on each side.
In published studies the numbers are different — one series with UPmax recorded an average of 43.6 mL (up to 60 mL), and a review of several studies arrived at about 207 mL — which shows that there is no standard volume. The full explanation is in how many mL the buttocks need.
How much does butt filler cost in Brasília?
Dr. Thiago's prices in 2026, in Brazilian reais (R$):
- Body hyaluronic acid: R$ 18,000 to R$ 55,000 per cycle, according to the amount of product.
- Biostimulator: R$ 2,900 to R$ 3,900 per box, with a minimum of 1 box per side; the ideal is usually 2 to 4 per side.
- Your own fat (Lipocube): R$ 30,000 to R$ 50,000 per protocol.
When the plan combines products, the amounts add up. Any total is an example, not a fixed package: the quote for your case comes out of the consultation. See the page on butt filler cost to understand what goes into the calculation.
What results can you expect, and when do they appear?
It depends on the product. Hyaluronic acid shows its volume right after the injection, and the body absorbs it little by little over time — which is why maintenance is part of the conversation. A biostimulator does not deliver immediate volume: the improvement in firmness comes gradually, as collagen forms.
No specific result can be promised. What can be offered is planning, an identified product, and follow-up. Below are three real cases from Dr. Thiago; the full gallery is in before and after.
Is butt filler safe?
No procedure is free of risk. What reduces risk is a sum of verifiable choices:
- Identified product: the boxes are shown and opened in front of you, and all the lot labels are affixed to your chart.
- No PMMA: Dr. Thiago does not use this material.
- Close follow-up: each and every complication is followed personally by Dr. Thiago.
Warning signs, PMMA, and how to find out what was injected into you are covered in safety and PMMA.
What is recovery like?
The procedure is performed in the office, under local anesthesia, and you go home the same day. Light activities are usually allowed right away; direct pressure on the buttocks is avoided in the first days, and weighted glute and leg training only resumes between day 21 and day 30, according to the instructions you receive. The details of each phase are in butt filler recovery.
How do you choose who will perform your procedure?
Five questions you can check before booking:
- Is the CRM (Brazilian medical license) active on the CFM portal? (Dr. Thiago's is CRM-DF 23199.)
- Is the training verifiable?
- Is there specific experience with the buttocks?
- Is the product identified in front of you, with a lot label?
- Who follows up on a possible complication?
Each criterion is explained in how to choose a doctor, and the background of the author of this guide is in about the author.
